Referring Physicians
Many health insurance plans (including Medicare) cover diagnostic tests. If your insurance requires a referral, please request your physician to fax it to our office prior to your appointment. If you are unsure your insurance requires a referral, please contact one of our offices for assistance. If we are still unable to determine if it’s required, we encourage you to request one from your physician.
